First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
If possible, please pre-register with our office by filling out our secure online Patient Registration Form and click the Complete and Send button at the bottom to automatically send us your information. If you prefer to bring the completed form with you to the office, simply click the download button and print the form. For returning patients, if you have not been to the office in a year, we ask that you please register again.
Please provide the following information at the time of your consultation:
- Your referral slip and any X-rays if applicable.
- A list of medications you are presently taking.
- If you have dental insurance, bring your insurance card and the necessary completed forms. This will save time and allow us to help you process any claims.
- Please do not take medication for discomfort 4-6 hours prior to your appointment, as this may mask your symptoms.
IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation.
Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e. antibiotics for pre-medication.)
As per the Massachusetts Department of Public Health, masks are now optional for patients in dental offices. We respect your choice to wear or not wear a mask when visiting our offices. Infection control has always been a top priority at our practice and we adhere strictly to guidelines recommended by the Occupational Safety and Health Administration (OSHA), the American Association of Endodontists (AAE), the American Dental Association (ADA), the MDS, and the CDC to ensure your safety. X-Ray
If your dentist or physician has taken x-rays and would like us to review them, they may forward them to our office. We take digital x-rays at our office for more detailed examination.
